How to submit forms

You may submit requested documents by using the fillable PDF form. Enter the data, then print and sign the form.

Forms can be mailed to the Financial Aid Office, Dyersburg State Community College, 1510 Lake Road, Dyersburg, Tennessee 38024, faxed to 731-286-3354, or submitted at any DSCC location.

Verification Deadlines

It is recommended that students submit requested verification documents within 30 days of the request. However, the final federal deadline to submit verification documentation is 120 days from the last date of enrollment for the academic year, or September 18, whichever is earlier. Students who fail to complete verification forfeit all federal aid and most state aid for the academic year.

The priority date for financial aid is August 1st for fall semester and December 1st for spring semester. Students need to provide the requested documentation to the Office of Financial Aid before these dates to ensure their financial aid is processed in a timely manner. Documents submitted after the priority dates will be processed, but financial aid may not be processed before the semester’s billing due date. Students are responsible for submitting documentation in a timely manner and paying the balance needed to secure their schedule.
